PSU Sponsored Short-Term Faculty and Student International Trips

As you already know, you are responsible for completing all PSU pre-departure requirements, prior to embarking on your international program. These requirements are essential for your safety and security while in another country. Should you have any questions about your pre-departure requirements, contact The Bagley Center immediately.

Steps to Complete Your Pre-Departure Requirements

  1. Read: PSU Short Term Pre-Departure Guide (PDF, 202 Kb)
  2. Review the U.S. Department of State Consular Information Web site for the country where you will study
  3. Review the Centers for Disease Control Web site for the country where you will study
  4. Print, Complete & Submit pre-departure paperwork. All forms are located in your PSU Short Term Pre-Departure Guide (PDF, 202 Kb)
    Paperwork includes:
    1. Basic Data & Emergency Contact Information – page 6
      *You should have submitted this form to the Bursar when you registered. If you haven’t already completed this form, you must submit it to The Bagley Center
    2. Release & Waiver of Liability – page 7
      *You should have submitted this form to the Bursar when you registered. If you haven’t already completed this form, you must submit it to The Bagley Center
    3. Authorization for Release of Medical Records – page 9
    4. Self Report on Health – page 10
    5. Agreement of Study Abroad Requirements – page 11
    6. Medical Evaluation – page 12
  5. Submit a clear copy of your passport – no faxed copies
All pre-departure requirements must be complete at least 3 weeks prior to your departure.

All participants on PSU Short Term Faculty and Student International Programs are enrolled in a comprehensive medical insurance plan.
