PSU Sponsored Short-Term Faculty and Student International Trips

As you already know, you are responsible for completing all PSU pre-departure requirements, prior to embarking on your international program. These requirements are essential for your safety and security while in another country. Should you have any questions about your pre-departure requirements, contact The Bagley Center immediately.

Pre-departure requirements outlined below include: Documents to Complete and Return to Bagley and Required Reading.

*All pre-departure requirements must be complete at least 3 weeks prior to your departure.

Documents to Complete and Return to Bagley

You have two options for completing paperwork: Fill-In or PDF format

  1. Fill-In format – Open document, fill out, print, sign and return to Bagley
  2. *If you choose to open the document and fill it in on the web, you cannot save what you have entered, even though you may print what you have typed. We recommend that you save the file to your computer before filling in the form and printing.
  3. PDF format - Open document, print, complete, sign and return to Bagley

***You must also submit a clear copy of your passport – do not fax a copy.

Required Reading

All participants on PSU sponsored international programs must read the PSU Short Term Pre-Departure Guide and completely review resources on the websites below. You are required to sign-off on the “Agreement of International Program Requirements” document, indicating that you have read the required material.
