If you are interested in having STD testing:

ü      Fill out the STD Questionnaire

ü      If you have answered any of the questions with a YES answer call Health Services at 535-2350 to make an appointment with the doctor or nurse practitioner.

ü      If you have answered all questions with a NO answer you can make an appointment with a nurse.  Call Health Services at 535-2350.

ü      Plan on 30 minutes for your appointment and bring your completed STD questionnaire with you.
