Your Health Records

Medical records are kept for a period of seven years. If you need your student medical sent to another location or received by us, you will need to grant permission to release/receive your records (PDF). You may also come in during business hours to sign the records release form.  You will need to specify your name, address, and the fax number of the individual or office the information will be sent to.

All of your visits to Health Services are kept strictly confidential.  Health Services has a legal and ethical responsibility to safeguard the privacy of all patients and to protect the confidentiality of their health information.
