Students, staff, and faculty can apply for passports Wednesday, Oct. 15 at the PSU Study Abroad Fair.
Staff of the U.S. State Department will be here to process applications from 11 a.m. – 2 p.m. at the HUB.
Advance registration is required.Visit the Bagley Center on Highland Street between 8 a.m. and 4 p.m. Monday through Friday.

WHAT DO I NEED TO GET MY U.S. PASSPORT?
NEW APPLICANTS:
"Application for a U.S. Passport" (form DS-11), available online at http://travel.state.gov.
Do not sign the application until asked to do so by an acceptance agent.
Two (2) recent 2" x 2" color photographs with plain white background.
* Proof of U.S. Citizenship. (e.g. certified U.S. birth certificate with seal, original Naturalization Certificate, Report of Birth Abroad, previous U.S. passport, etc.)
Important note: birth certificates with notary seal(s) and photocopied birth certificates will not be accepted. Seek guidance from the Passport Center staff to assist applicants.
Valid form of photo identification (driver's license, government issued ID, or military ID).
NOTE: You will need to appear in person if you are applying for a U.S. passport for the first time; if your expired passport is not in your possession; if your previous passport is expired and was issued more than 15 years ago; or if your previous passport was issued when you were under 16 or if your passport has been stolen.
For minors under the age of 16:
Each minor child shall appear in person.
All applications for children under 16 require both parents' or legal guardians' consent. (for more information, please visit http://travel.state.gov. For all minors ages 16 & 17:
Each minor child shall appear in person.
For security reasons, parental consent may be requested.
If your child does not have identification of his/her own, you need to accompany your child and present identification.
RENEWAL OF A U.S. PASSPORT:
You can renew your U.S. Passport by mail if the following guidelines are all met:
Your most recent passport is available to submit and it is not damaged;
You received the passport within the past 15 years;
You were over the age of 16 when it was issued;
You still have the same name, or can legally document your name change.
Required items for renewal by mail:
Application for U.S. Passport by Mail (form DS-82)
Your most recent U.S. Passport
Two (2) recent color 2" x 2" photos
Marriage certificate or court order if your name has changed.
The Passport Card
Starting in June 2009, you will need a passport or passport card in your possession when you drive across our borders to the north or south or travel by ship to Bermuda and the Caribbean.
The new Passport Card not only costs less than a Passport, but also makes it a lot easier to have proof of citizenship for travel between countries on the North American Continent by land or sea.(The passport card is insufficient for international air travel)
