Directions for Retrieving Forms

  1. Go to the OTC Forms Page
  2. Once the forms page has loaded, scroll down and click on either Student Teacher Observation Form, or for a midterm or final, click on the Evaluation of Student Teaching Experience form that is appropriate for your specific program. These forms are all located under the heading of Forms for Clinical Faculty, Cooperating Teachers and Student Teachers.
  3. The form will open on another page.
  4. Click on the save icon located in the upper left corner of the document. You must save the form on your computer to either your desktop or documents folder.
  5. Click Save.

“I love the connections that I have made and the opportunities that I have been presented with. With people that actually care and want to see me succeed it makes it so much easier to succeed. I have made some great connections with faculty, staff, and classmates that I will never forget. Everyone is so friendly and welcoming that it was so easy to find my place here at Plymouth.”

What do you like best about your major?

“I love my professors and how interactive all of my classes are. Through my program I have made so many connections and have been able to apply what I learn in the classroom to the real world. I have been provided with many opportunities for internships and practicum experiences, which have been amazing.”
