New Students

A medical physical exam is not required for admission to PSU. If you are planning on participating on a sports team, contact the Athletics Department for their requirements.

Completion of the Health Questionnaire (PDF) is required. No immunizations are required, although a list of immunizations received is requested. Hepatitis B and Meningococcal vaccine, Varicella and Human Papilloma virus vaccine are strongly recommended.
