Submitting Proof of Vaccination
Employees and students who are fully vaccinated may voluntarily submit their proof of vaccination to be excused from the weekly COVID-19 testing. You may choose which option below works best.
Please submit a copy of your vaccination card or certificate online using the form below.
Vaccinations cards or certificates may be submitted in-person to Human Resources Monday through Friday, 9am-3pm. Proof of vaccination may also be brought to any on campus testing event.
If you would like to mail a copy of your vaccination card, they may be sent to the following address.

There could be a delay in the mail arriving, so we ask you to send in a copy of your card a week ahead of the class meeting.
Emailing a copy of a vaccination record or certificate is an option the University has made available, however it is important to note that the COVID-19 email is not HIPAA compliant. By emailing your vaccination card, it combines health and non-health information in a single email inbox. If this method for submission is selected please send to: COVID19@plymouth.edu. Please include your first and last name, date of birth, and your PSU ID in the body of the email.
