Reporting at PSU

The reporting options below give community members the opportunity to share important information regarding observed behavior that raises concerns related to various incidents occurring on or off campus involving PSU community members. The intention for response is to address concerns in a safe, timely, and supportive manner, with the goal of connecting individuals to the resources they need.

Emergencies: Incidents that require immediate attention should be reported to University Police by dialing 603-535-2330 or 911.

Anonymous Reporting: Plymouth State University is committed to providing a safe, timely, and supportive response to all incidents. This often requires follow up with those directly impacted by the behavior. Anonymous reports are accepted, though they may limit the institution’s ability to respond. When reaching out to individuals impacted by incidents, the University prioritizes the rights of the involved parties in determining how to proceed.

Accident Report for Students and Guests

Use this form for reporting any injuries, beyond those that can be resolved with a first aid kit, involving PSU students or guests. This form is received by staff at Facilities Services to address infrastructure needs and safety concerns. Completing this form does not address imminent health or safety concerns. If you are experiencing an emergency, please contact UPD as noted above or by dialing 911.

Bias Incident Report Form

Plymouth State University is committed to maintaining a positive campus climate for all community members. By reporting incidents that have negatively impacted you, others, the community, and/or are contrary to the Plymouth State University values and codes of conduct, you are making a valuable contribution in helping to maintain the integrity and safety of the campus community as a whole.

C.A.R.E. Form

Use this form to inform the Frost House when a student may be in need of assistance, such as a significant incident impacting their ability to succeed - temporarily or long term - in the campus environment. This form is received by Frost House Student Services.

Discrimination and Discriminatory Harassment

Use this form to disclose incidents involving discrimination or discriminatory harassment (behavior directed at an individual on the basis of a protected class or status that impacts their learning or work environment). This form can be used by third parties or those directly impacted by the behavior. Completing this form does not constitute taking the step to submit a formal complaint. This form is received by the Title IX Coordinator.

Sexual Harassment/Violence

Use this form to disclose incidents involving sexual harassment (sexual assault, harassment, stalking) and relationship violence. This form can be used by third parties or those directly impacted by the behavior. Completing this form does not constitute taking the step to submit a formal complaint. This form is received by the Title IX Coordinator.

Silent Witness Program

Submit a confidential tip form. After hours, University Police telephone lines are answered by the Plymouth Police Communications Center which is staffed 24 hours a day.
