Alumni Recognition Awards Nomination Form

Each year at Homecoming, the Plymouth State Alumni Association recognizes alumni for their service to their community and the University. We thank you for taking the time to nominate a deserving alumna or alumnus for recognition. Self-nominations are welcome and encouraged. Nominations will be accepted until May 31, annually.

**Please note that University Advancement employees, current PSU Alumni Association Board of Directors, and current President's Council members are ineligible to receive Alumni Recognition Awards. Previous award recipients are also ineligible.
