How to Apply/Forms

All Plymouth State University faculty, staff, and students who plan to conduct research involving human subjects must submit applications to the IRB prior to commencing the study.   Applicants must receive written, unconditional IRB approval before starting their study. Beginning on October 14, 2026 applications must be submitted to the IRB for review via Cayuse IRB/Human Ethics.  NB: All applications before that date will be reviewed as submitted and will not need to be resubmitted. 

All non-faculty applicants will need to request from the team at UNH that a Cayuse account be created prior to applying. 

Anyone needing assistance with the Cayeuse software should contact the UNH IRB as they are the “owners” of the software and experts on its functioning.  They’ll be able to efficiently problem solve those types of issues with you.
