The Plymouth State University C.A.R.E. Program provides holistic support through early intervention and resource referrals, helping connect students with appropriate campus and community services to promote their success at PSU.
The program is overseen by a multidisciplinary team that meets regularly to maintain a safe and supportive learning environment. The C.A.R.E. Team evaluates reports of concern, coordinates appropriate interventions, and makes recommendations for further action when needed.
Reporting Student Concerns
Faculty, staff, students, and family members are encouraged to report concerns using one of the two designated reporting forms:
Academic Concerns Reporting Form
Use this form to report academic performance concerns such as frequent absences, missed assignments, low grades, or signs of disengagement. Reports may result in outreach, tutoring referrals, advising support, or other academic success services.Social/Emotional C.A.R.E. Reporting Form
Use this form to report non-academic concerns such as significant emotional distress, behavioral changes, traumatic events, or signs of potential harm to self or others. Submissions may result in mental health referrals, wellness checks, peer support, or coordination with appropriate university department.
Purpose and Process
The C.A.R.E. Program acts as a central hub for collecting and sharing important information about student well-being. This proactive approach helps the university manage complex situations involving personal safety, emotional well-being, behavioral concerns and academic performance.
Key objectives of the C.A.R.E. Team include:
- Identifying and assessing potential threats or safety concerns
- Identifying and assessing barriers to academic success
- Coordinating appropriate interventions and referrals
- Supporting the safety and well-being of the university community
Commitment to Privacy
All C.A.R.E. Team members are committed to protecting the privacy of those involved. Reports are handled with the highest standards of privacy and professional discretion.
Confidential Resources for Students
The Plymouth State University Counseling Center is a confidential resource for students. For more information or to schedule an appointment, call the PSU Counseling Center at (603) 535- 2461.
Voices Against Violence is the local crisis services agency offering confidential advocacy for matters involving sexual and interpersonal violence. Contact an advocate at (603) 536-5999 or 24/7 crisis line: (877) 221-6176. Confidential resources will not report to law enforcement or university officials without the consent of the person who supplied the information, except for in extreme circumstances involving life safety.
Emergency Protocol
If a student is believed to be in immediate danger to themselves or others, the Executive Director for Student Advocacy, Well-Being, and Crisis Intervention, or their designee, reserves the right to contact a parent or guardian if emergency services have not already done so.
