Reporting an Incident

Students and employees are strongly encouraged to report incidents of sexual misconduct, discrimination, discriminatory harassment, and related retaliation to the University.

Forms are submitted to the Title IX Coordinator for review and response. Form submission does not automatically constitute filing a formal complaint. Individuals will be provided with outreach about rights and resolution pathways available.

Support and Resolution Options are available: The University offers support-based, agreement-based, and investigation/decision-making based resolution options. Supportive measures may be arranged through the Title IX Coordinator with each resolution pathway.

Confidential Advocacy Available 24/7: For incidents involving sexual assault, sexual harassment, stalking, and relationship violence, confidential advocates from Voices Against Violence are available twenty-four hours a day, seven days a week. Advocates support clients with seeking medical attention after an incident, safety planning, consulting with the Title IX Coordinator, reporting to law enforcement, obtaining civil protective orders through the court, applying for victim’s compensation for reported crimes, and accessing other resources.  Voices Against Violence, our local, off-campus crisis services agency is PSU’s primary resource referral for individuals who have experienced sexual violence. Reporting to Law Enforcement: Individuals may also choose to report crimes to law enforcement. The Title IX Coordinator can help arrange initial contact with University police or local police to report or to learn more.
